The following forms are to assist you in preparing for your massage. I have all new clients fill out a client information form. If you are pregnant, I shall have you fill out a pregnancy/prenatal form as well. All clients under the age of 18 must have their parent/guardian fill out and sign a consent to treat minor form. If you have any contraindications, please have your physician fill out an sign the physician permission form. All client information is kept confidential.
